Service Agreement & Damage Waiver
By checking the box below, I authorize Extreme Audio of Hinesville to perform the requested installation, service, and/or repairs on my vehicle.
I understand and agree to the following:
• Labor Warranty: Installation labor is warranted for as long as I own the vehicle. This warranty covers defects in workmanship only and does not apply to issues caused by misuse, tampering, modifications, accidents, or work performed by other parties.
• Additional Charges: Additional charges may apply if unforeseen parts, wiring, fabrication, fitment, filters, wiring harnesses, or additional labor are required during installation. Engine component or hose removal may be necessary in some cases. Customer authorizes Extreme Audio of Hinesville to contact them for approval of any additional parts or labor needed to complete the service.
• Returns & Restocking: No refunds are provided on installed, special-order items, or window tint deposits. Unopened items may be eligible for store credit only and may be subject to a 20% restocking fee.
• Vehicle Modifications: Installation may require removal or modification of factory panels, trim, wiring, or components. While care is taken, minor cosmetic changes may occur.
• Pre-Existing Conditions: I acknowledge that my vehicle may have pre-existing conditions (including electrical issues, prior damage, or worn components), and Extreme Audio is not responsible for these conditions.
• Personal Property: Extreme Audio is not responsible for loss or damage to personal items left inside the vehicle. I agree to remove or secure all valuables prior to service.
• Legal Compliance: I understand that certain equipment (including video systems) may be restricted by law while the vehicle is in motion, and I assume full responsibility for compliance with all applicable laws.
• Payment Terms: I agree to pay in full upon completion of services unless otherwise agreed in writing. By signing below, I acknowledge this agreement and agree not to dispute charges for authorized work performed.
• Payment Authorization: I authorize Extreme Audio of Hinesville to charge my payment method for the approved services, including any additional charges I approve during the job.
